Specialist Community Practitioner (Development Role 6-7)

Jean Bishop Frailty Community Services (JB-CFS) have been operational since May 2018, and in that time provided innovative, anticipatory patient tailored care to frail residents of Hull and the surrounding areas, including those residing in care homes.

The Centre has already been recognized for national awards and continues to receive outstanding patient feedback. The JB-CFS also featured on NHS England's website dedicated to the NHS Long Term Plan as the leading case study in the Ageing Well section. More recently, the team featured on a Panorama programme which assessed the innovative and new ways of working delivered by our team, evidence to support not only our frail population but also the demands placed on the NHS.The team working at the JB-CFS feel privilege to be a part of delivering transformed care for older people in Hull.

In this role you will be working in providing urgent medical care to frail patients acutely unwell in their usual residence, including care homes. This is an amazing varied opportunity for an independent prescriber with advanced practice qualifications who is passionate about caring for older and frail patients, who also has the drive to help further develop a high profile service care within the community.

Main duties of the job

In this role you will work as an expert autonomous practitioner supported by the JB-CFS; assessing and delivering the care and treatment of older people and those with frailty, working closely with the multi-disciplinary team and community services to ensure patients receive holistic and comprehensive care and support.

You will need to possess the necessary skills to deliver services to patients with a wide range of complex clinical needs, in particular acute presentations. Experience of working with patients suffering long term conditions would also be advantageous. The abilities to communicate professionally and effectively whilst in a caring manner with patients and their carers together with experience of working within a multi-disciplinary team are also essential, as is working experience/knowledge of local pathways and referral procedures.

Qualifications
  • One of the following: o First Level Registered Nurse o Registered Allied Health Professional
  • A post registration qualification in one of the following: o Specialist Practitioner (diploma / degree / MSc) o Community Practice o Autonomous Practitioner module/first contact physical assessment skills. (level 6 or 7) o Degree level qualification in clinical practice relevant to this role o Advanced history taking and Physical Assessment
  • Independent non-medical prescriber V300
Knowledge
  • Current legislation and impact on community nursing services
  • Evidence based practice
  • Evidence of understanding of the virtual ward concept
  • Demonstrate an understanding of proactive caseload management
  • Understanding of audit processes, Key Performance Indicators and Targets, CQUIN collection data and Quality Monitoring Programmes

Experience
