A national healthcare provider in Scotland is seeking a Specialist Clinical Pharmacist to lead an evolving Integrated Front Door team. The role involves patient care within the Hospital at Home service and supporting Urgent Care and Acute Assessment teams. Applicants must possess excellent communication skills and a commitment to innovation in healthcare. This is a permanent position offering a salary of £50,861 - £59,159 plus allowances.

We have an exciting opportunity for a Specialist Clinical Pharmacist to join an evolving Integrated Front Door team. We are seeking a motivated and enthusiastic pharmacist to lead on expansion of our Integrated Front Door service, encompassing Hospital at Home, Acute Assessment Unit, Urgent Care and Frailty, shaping the way rural healthcare is delivered within the remote‑island location.
This post will cover patients within the Hospital at Home virtual ward as well as providing support for the Urgent Care and Acute Assessment teams. In addition, the post holder will be responsible for implementing and delivery polypharmacy reviews across the services and in particular working with the new Frailty team as part of the MDT performing Comprehensive Geriatric Assessments. There is a vision to expand the service in line with Scottish Government initiatives. Applicants should be committed to providing a high standard of patient care and interact well within the multi‑disciplinary Team.
Applicants should have good communication skills, and a commitment to developing change and promote innovation within the front door services. Post holders are required to hold a driving licence.
Working hours will be Monday – Friday 0900-1700.
The ability, drive and commitment to work as part of a team, sometimes at a distance, and to contribute to the local development of primary care and acute services is important.
