A local public service organization is seeking an Experienced Social Worker to join their Transfer of Care Hub. The role involves proactive support for safe discharges from the hospital, working collaboratively with health and social care teams to ensure smooth transitions for patients returning to the community. Candidates must have social work experience and strong communication skills, with DBS clearance required. Our Transfer of Care Hub is a dynamic, multidisciplinary team that supports safe, timely and person centred discharges from hospital. We work collaboratively with acute, community social care and voluntary sector partners to ensure that patients received the right care, in the right place at the right time.
Our focus is on reducing delays, improving patient flow and delivering high quality Discharge to Assess (D2A) and Home First pathways. We are committed to delivering excellent standards of communication, proactive problem solving and strong partnership working to improve outcomes for the people we serve.
This post is based at Warrington General Hospital, reporting the the operational team manager.
As an Experienced Social Worker, you will be a valued member of the team, working with health and social care colleagues, carers and families andother statutory and voluntary agencies to identify and plan the support people requireto return to the community. Our team is solution focused placing the needs of theperson at the heart of our decisions, ensuring they have the support they require to notonly return to the community but also to live their lives well and independently.
You will provide proactive and responsive support in the discharge process, working within the scope of the Hospital Discharge and Community Support Operational Policy Guidance, from within the Transfer of Care Hub. Working closely with service users, carers, ward‑based staff, Primary Care, Community Health, Social care, and the voluntary sector. Supporting the patients at ward level to ensure effective discharge planning processes are followed in a timely manner for the safe discharge of patients from hospital.
Collaborative working and the ability to create and maintain effective working relationships with a range of health and social care colleagues both in the hospital and the community are an essential element of the role.
This role is predominantly basedon-site, however some hybrid working is available.
