Main Duties

• Support referrals for your assigned area; conduct comprehensive assessments and develop perinatal specialist mental health care plans.
• Monitor and support women during pregnancy or post-natally; maintain links with health providers (Midwives, Specialist Midwives, Health Visitors, etc.).
• Plan CPA reviews; attend and cover team duties, huddles, meetings, and provide buddy support/lone working as needed.
• Support colleagues with complex cases, escalate safeguarding concerns, and ensure safety plans for high-risk service users.
• Stay up to date with perinatal research.
• Plan and deliver direct patient care with minimal supervision within a multi-disciplinary and multi-agency environment.
• Manage caseloads and provide mental health care services to individuals with mental health needs.
• Record accurate mental health information at admission, during review, and on discharge.
• Work within the multi-disciplinary team and collaborate with other care providers.
• Empower service users and carers to make informed decisions about care, coordinating with agencies inside and outside the team.

Requirements

• Degree or equivalent in social work
• Current Social Work England registration
• Significant frontline experience in Adults Services within the UK
