Overview

Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for an Adult Social Worker within the Central Locality Mental Health Team to work Full Time based in Haringey.

The salary for this permanent Social Worker job is up to £52,194 per annum.

Responsibilities
  • To take responsibility for a suitable caseload and develop experience and competency across the following duties with opportunity to progress from one grade to the next when/if progression requirements are met.
  • Develop and maintain effective relationships with service users, families, carers and communities to help them make informed decisions, enabling them to clarify and express their needs and contribute to service planning that is person-centred and strengths based.
  • Work effectively within legal, policy and financial frameworks, obligations, structures and culture, in particular, the Care Act, Mental Capacity Act, Mental Health Act, Human Rights and Equalities legislation and accompanying guidance and codes of practice.
  • Be responsible for the assessment and safe discharge of service users from hospital back into community settings, working within challenging timeframes and reducing risk of future re-admission.
  • Undertake Mental Capacity assessments for service users where it is deemed that they may lack the mental capacity to make a specific decision.
  • Identify service users who are eligible for Continuing Health Care (CHC) or Section 117 funding and undertake checklists and work with other professionals to provide a multi-disciplinary assessment within this framework.
  • Complete Human Rights Act assessments for those who have no recourse to public funds but may require support under the Care Act.
  • Take responsibility for managing an individual caseload of clients, regularly monitor, review and evaluate changes in clients needs and ensure that all services contribute effectively to the support/care plan, and it remains an effective use of resources.
  • Undertake ongoing and urgent case reviews as necessary.
  • Intervene in emergency situations to protect vulnerable adults and children and initiate the appropriate statutory or other actions required.
  • Participate in office duty on a rota basis, responding to new referrals, assessing urgency of requests and implementing actions to meet immediate needs.
  • Develop and maintain up to date relevant knowledge and competency and personal effectiveness in relevant legislation, social work practice and changes in social work including strengths-based working delivery models through development as agreed by the Team Manager.
  • Fully participate in individual supervision and performance appraisal as well as participate in the development and implementation of performance indicators and quality service standards, always striving for improved performance both personally and as a service.
