Skills and Qualifications:

* Essential: Degree in Social Work with current Social Work England registration.

* Experience: Previous experience working in a hospital discharge team or adult social care setting.

* Skills: Excellent communication, analytical, and report-writing skills, with the ability to manage complex caseloads.
