Full-time positions are available for those looking to make a positive impact in the community by facilitating smooth transitions for adults moving from hospital to home environments.

Responsibilities
  • Support patient flow from psychiatric units to ease hospital discharge pressures and facilitate movement from step-down arrangements.
  • Address complex care needs for individuals over 65 to promote successful reintegration into the community.
  • Assess individual needs and contribute to support planning and co-design.
  • Ensure regular monitoring and review of support plans for continuous improvement of wellbeing and outcomes.
  • Manage your caseload independently while upholding department and council guidelines and maintaining a positive departmental reputation.
Job purpose

You will play a crucial role in the operational delivery of Adult Social Care Services, working within an integrated team to safeguard and support adults with varying care needs. Your efforts will directly contribute to improving individual outcomes and promoting overall wellbeing.

Requirements
