Key Responsibilities
  • Support service users who have been discharged home from hospital with ICT short‑term care.
  • Complete social care assessments, mainly via telephone, to identify long‑term needs.
  • Create clear, person‑centred care plans to enable service users to move to long‑term providers.
  • Work in a fast‑paced environment managing a steady flow of referrals.
  • Carry out occasional in‑person MCA assessments where required.
  • Maintain accurate case notes and follow statutory guidance.
Requirements
  • Experience in adult social care or hospital discharge pathways.
  • Strong assessment and care‑planning skills.
  • Confident completing telephone‑based assessments.
  • Understanding of the Mental Capacity Act.
