A local authority in the UK is seeking a dynamic Social Worker for its Active Recovery Community Home team. The role focuses on hospital discharge support and requires strong collaboration with health and care partners. Ideal candidates are qualified social workers with MDT experience and a commitment to high-quality service delivery. Social Worker (Active Recovery Community Home team hospital discharge team) – SO3 (£39,862 - £45,091). This role is eligible for an Essential Car User allowance of up to £963 per annum. 37 hours, Full-Time (8:30am–5:00pm, Monday to Friday). Permanent. Hybrid: Stepping Hill and homeworking.
