What you will do:

• Conduct statutory Child and Family assessments in accordance with CA 89 and the Chronically Sick and Disabled Act.
• Develop and chair the implementation of Child in Need and Child Protection plans as required.
• Engage effectively with children and young people facing communication challenges.
• Evaluate risks to children with disabilities and propose sound protection measures.
• Identify suitable short break services for children with disabilities.
• Manage Children in Care, ensuring care plans are comprehensive and current.
• Conduct court work when necessary, ensuring the best outcomes for the children and their families.
• Facilitate the transition for young adults into adult services, crafting Transition Plans that meet their needs.
