Job description

The Social Worker Pact Aligned Care Team Position

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Location: Bedford VA Medical Systems, Bedford, Massachusetts.

Hours: Full time, 40 hours per week.

The Social Worker Pact Aligned Care Team position is in Social Work services at the Bedford VA Medical Systems located at the Bedford, Massachusetts location. The PACT Social Worker serves as a member of the PACT team providing a variety of clinical interventions to Veterans in the primary care setting. PACT social workers assist with care coordination and access to care for Veterans.
