The role involves Care Act assessments, reviews, Mental Capacity Assessments (MCA), Continuing Healthcare (CHC) work, and participation in a duty rota. What you will do
  • Provide a professional social work service to adults within a multi-disciplinary team.
  • Carry out the statutory duties of the Directorate in relation to adults, following all policies and procedures.
  • Manage your own caseload under the supervision of the Team Manager.
  • Undertake social work tasks closely with other professionals to assess needs and deliver care packages.
  • Develop skills in working with older adults, including those with mental infirmity and physical disabilities.
  • Collaborate with team members and assist them when needed.
  • Cultivate special interests aligning with service priorities.
  • Forge links with other agencies, including statutory, voluntary, and community groups.
  • Participate in the duty system during office hours on a rota basis.
  • Engage in any other duties relevant to the role.
  • Use IT applications effectively for inputting, recording, and communicating information.
Compliance Requirements

Enhanced DBS Check (with Adults Barred List), HCPC Registration, Verified 2-year referencing with all gaps accounted for, Photographic ID, Signed Confidentiality Agreement, Eligibility to work in the UK confirmed, Full work history with no unexplained gaps.

Candidates must have a CQSW/DipSW/CSS qualification and relevant experience in social work intervention, assessment, and care planning. A good grasp of current legislation, strong interpersonal and communication skills, team work ability, and IT proficiency are required. You must also demonstrate an understanding of Equal Opportunities Policy and its application in practice. Flexibility and adaptation to changing work practices are crucial. A willingness to obtain or enhance qualifications and training is essential, as well as the ability to meet travel requirements as needed.
