As a Social Care Practitioner, you will play a vital role in supporting adults to regain independence following illness, injury, or a change in circumstances. Working closely with individuals, families, and multi-disciplinary professionals, you will assess needs, develop person-centred plans, and provide short-term interventions that empower people to live safely and confidently in their own homes.
