Hours per week: 37.5

Contract type: Permanent

This is a Permanent, Salaried vacancy that will close in 22 days at 23:59 BST.

The role

As a Service Coordinator you’ll support customers who have complex needs and/or are experiencing multiple disadvantage including: substance use, mental health, offending and rough sleeping/homelessness. You’ll coordinate their access to a wide range of services and lead on the design and delivery of a service coordination plan alongside the customer and other essential agencies.

Job Requirements
  • Provide flexible, compassionate support to customers with complex needs, including homelessness, substance use, mental health.
  • Empower customers to regain independence and help them understand what it takes to manage their own tenancy.
  • Work closely with the multi-agency team and support network to strengthen support.
  • Develop and design tailored service/support plans.
  • Keep clear, accurate records and stay up to date on housing and safeguarding laws.
  • Work alongside the Support Team Leader to promote Concrete’s services, build community relationships and monitor service impact.
What We’re Looking For
  • Experience supporting adults with complex needs such as substance misuse, homelessness and ill mental health.
  • Experience working with confidential and complex information, following data protection laws.
  • Genuine compassion for people affected by homelessness and an understanding of the barriers they face.
  • Have an understanding of all Microsoft applications including Outlook, Word, Excel and Access.
  • Great communication and problem-solving skills.
  • Strong organisation and time management skills and the ability to prioritise high-risk cases.
  • Full UK driving licence with own vehicle for work purposes is essential.
  • DBS check is required.
