Job summary

A regional health authority in Torquay is seeking a Senior Social Work Practitioner for a 12-month secondment in the Mental Health Adult Social Care Team. The part-time position involves leading holistic assessments and managing complex cases to deliver effective services for working-age adults. The ideal candidate will have AMHP status and knowledge of relevant social care legislation. Qualifications

Job description
Senior Social Work Practitioner – Secondment

12-Month secondment or fixed term opportunity for a Senior Social Work Practitioner in the Mental Health Adult Social Care Team.

Part‑time 35 hrs per week, Band 7 with an 11.8 % salary enhancement for qualified AMHPs.

Responsibilities
  • Core member of a multidisciplinary mental health team, contributing to the effective provision of services to working‑age adults in Torbay.
  • Lead holistic social care assessments, partner with service users, families and carers, prepare care plans, commission services and ensure cost‑effective care packages are established, delivered, monitored and reviewed in accordance with all statutory requirements.
  • Safeguard adults and children, practice social care legislation including the Care Act 2014 and the Mental Capacity Act 2005.
  • Jointly manage resources and relationships with key agencies to deliver timely and effective assessment and personalised support planning.
  • Provide leadership and management to a multi‑disciplinary team, support practitioners in risk assessment and risk management, and offer shared case management for highly complex cases.
