A prominent healthcare provider in Farnham is seeking a Senior Registered Nurse for the Living Well service to deliver palliative care and support patients in the community. The ideal candidate will have experience in palliative care and must hold an RGN qualification with current NMC registration. This role requires both leadership in patient care and collaboration with multidisciplinary teams to ensure effective patient support. Excellent communication and interpersonal skills are essential for success in this role.

Salary: £42,035 per annum WTE based on 37.5 hours a week
Hours: Full and part time hours available

Our Living Well service is looking to appoint a Senior Registered Nurse with experience in both palliative care and working within the community. This pivotal role will support our patients and those closest to them, equipping them to manage the impact of their illness and remain as independent as possible while maximising wellbeing and quality of life.
The Senior Registered Nurse role will work across our outpatient and community locations, working closely with other multidisciplinary team members to proactively monitor and support patients, providing timely input to those with stable and slowly changing needs. They will also work in partnership with internal and external healthcare professionals as well as patients and their families, ensuring person centred, effective and co-ordinated care.

Overview of duties include working as a proactive member of the Living Well multidisciplinary team and providing nursing care for patients in the community, outpatient settings and across services as required.
