Overview

Senior Registered Nurse Living Well

Salary: £42,035 per annum WTE based on 37.5 hours a week

Hours: Full and part time hours available

Location: Farnham/Guildford, Surrey

Our Living Well service is looking to appoint a Senior Registered Nurse with experience in both palliative care and working within the community. This pivotal role will support our patients and those closest to them, equipping them to manage the impact of their illness and remain as independent as possible while maximising wellbeing and quality of life.

The Senior Registered Nurse role will work across our outpatient and community locations, working closely with other multidisciplinary team members to proactively monitor and support patients, providing timely input to those with stable and slowly changing needs. They will also work in partnership with internal and external healthcare professionals as well as patients and their families, ensuring person centred, effective and co-ordinated care.

Responsibilities

Overview of duties include working as a proactive member of the Living Well multidisciplinary team and providing nursing care for patients in the community, outpatient settings and across services as required.

  • Provide face-to-face services of planned visits to people with palliative care needs.
  • Support patient-centred palliative rehabilitation to maximise quality of life and independence.
  • Identify condition changes promptly and manage symptoms with proactive escalation as needed.
  • Manage caseload in the community and provide support in outpatient and group settings for patients and carers.
  • Collaborate with colleagues to process referrals, handle professional and patient enquiries, prioritise workload, and respond to clinical need in a timely manner.
  • Provide leadership for junior nurses and rehabilitation assistants within the Living Well team.
  • Where appropriate, work across services including the inpatient unit to meet patient needs.
About You

A successful Senior Registered Nurse will have:

  • A RGN Degree Level Qualification
  • A Current NMC registration in adult or general nursing
  • Palliative / end of life care or long-term conditions experience
  • A qualification in palliative care, or be willing to work towards one
  • A full driving licence and be willing to drive their own car
  • Experience of multidisciplinary team working, and with external partners
  • The ability to make autonomous decisions demonstrating critical thinking and capacity to proactively manage a caseload
  • Excellent communication and interpersonal skills
  • The ability to work collaboratively as part of a team
  • Previous line management experience would be an advantage
