Overview

Three-month locum contract for a Senior Practitioner in Social Work within the Integrated Learning Disabilities Team, Hackney. Primary focus on supporting adults transitioning to greater independence.

Responsibilities
  • Work within the Preparing for Adulthood Pathway, supporting individuals aged 18-25 transitioning towards greater independence and fulfilling their potential.
  • Engage with a multidisciplinary team to assess, plan, and implement tailored care packages, ensuring quality and effective service delivery.
  • Develop and maintain robust support networks that include family, friends, and community resources to facilitate holistic support for service users.
  • Utilise a strength-based and trauma-informed approach to practice, promoting inclusivity and understanding the complex needs of clients, including their intersectional identities.
  • Manage positive risk-taking, encouraging individuals to explore opportunities while ensuring safety and well-being.
