Job description

This position qualifies for a market supplement payment of £200 per month. Please note this additional payment will be reviewed on 31 March 2026.

The successful candidate may be eligible to up to a maximum of £8,000 (including VAT) towards the cost of expenses incurred due to relocating to Ceredigion. An exciting opportunity has arisen within Ceredigion’s Integrated Triage and Assessment Team to develop a dynamic, hospital-based social work service.

Key Responsibilities
  • Act as the key point of contact for hospital referrals, offering expert advice, guidance, and escalation support to health colleagues.
  • Lead on complex assessments, including Decision Support Tools (DSTs), Mental Capacity Assessments, and Best Interest Decisions.
  • Coordinate and supervise social workers in assessment and discharge pathways.
  • Coordinate and supervise Social Work Assistants to support low-level assessments and the Discharge to Assess (D2RA) model.
  • Facilitate pre-referral discussions to reduce delays and prevent unnecessary referrals to social care.
  • Support a collaborative framework for assessments linked to the Targeted Care & Enablement Service and utilising trusted assessor role.
  • Supporting pre-admission assessment for Ceredigion County Council residential services as a trusted assessor.
  • Enable a front-door turnaround approach, ensuring timely access to care and support plans and follow-up assessments in the community.
  • Build strong working relationships with health professionals to support effective decision-making and pathway planning.
What We’re Looking For
  • An experienced Qualified Social Worker registered with Social Care Wales.
  • Experience in hospital discharge planning, complex assessments, and multi-agency working.
  • Strong leadership and coordination skills.
  • Knowledge of relevant legislation including the Social Services and Well-being (Wales) Act 2014.
  • Ability to work autonomously and collaboratively in a fast-paced environment.
