Job summary

A leading healthcare provider in Greater London is seeking a Senior Physiotherapist/Occupational Therapist for their Community Independence Service. The ideal candidate will be responsible for delivering skilled care to clients at home, focusing on evidence-based assessments and interventions. Qualifications

  • Extensive post qualification experience of client care in an acute or community setting.
  • Evidence of working in a multidisciplinary team.
  • Knowledge of current national guidelines on hospital discharge process.

Responsibilities

  • Assess and plan interventions to optimise patient safety post-discharge.
  • Prioritise client care factors based on clinical reasoning.
  • Coordinate care with external agencies and community colleagues.
