About Provide Stroke Rehabilitation Service

Provide Stroke Rehabilitation Service supports 25 inpatient stroke beds and a well‑established Early Supported Discharge Service. We partner with Acute Stroke Units, Social Care and the Stroke Association to deliver seamless, high‑quality stroke rehabilitation. Our Nursing and Therapy teams, supported by Stroke Psychologists and Medical Practitioners, provide a progressive service showcasing true multi‑interdisciplinary working. Our philosophy encourages staff participation at all levels and identifies opportunities for service and career development.

Job Summary

This is an exciting opportunity to join a team of stroke‑skilled professionals to create a seamless end‑to‑end pathway for stroke patients. The post holder will work across Early Supported Discharge (Community Stroke) and the Stroke Rehabilitation Unit in Brentwood, providing an excellent chance to work across the community pathway and focus on the delivery of the ICSS model 2022.

Key Responsibilities
  • Work as part of an integrated multi‑disciplinary team within the Stroke Rehabilitation service based at Brentwood Community Hospital.
  • Promote, maintain and develop a high‑quality, efficient and effective Physiotherapy service for patients referred to the Mid‑Essex Stroke Rehabilitation service, in accordance with team philosophy and operational policies.
  • Manage a complex caseload using knowledge‑based clinical reasoning, critical thinking, reflection and analysis to support assessments and a client‑centred, goal‑orientated rehabilitation programme.
  • Participate actively in the stroke team, specific clinical projects, and service development, audit and review activities as agreed with the Team Leader.
  • Provide supervision to therapy assistants and physiotherapy students during on‑placement in the service.
  • Contribute to induction and training for junior staff, therapy assistants, students, other disciplines and carers.
  • Engage in Continuing Professional Development.
