Job summary

Qualifications

• Demonstrable experience in assessing and managing acutely unwell patients.
• Experience in community, urgent care, or primary care settings.
• Full UK driving licence and access to a vehicle.

Responsibilities

• Provide same-day and urgent clinical assessments for housebound patients.
• Deliver proactive and reactive care to residents in nursing and residential homes.
• Work collaboratively with GPs, Practice staff, ARRS roles, community nursing teams, and social care.

Skills

Registered healthcare professional NMC or HCPC

Excellent communication and organisational skills

Advanced clinical assessment skills

Ability to work autonomously and exercise clinical judgement

Education

Qualified ACP, Paramedic, or Registered Nurse

Independent prescribing qualification V300 or equivalent
