Senior Clinical Pharmacist: Crisis Resolution Home Treatment Teams

To actively support the development and delivery of pharmacy services to the Trust's Crisis Resolution and Home Treatment Teams (CRHT). To act as a non-medical prescriber in the CRHTs. To ensure that the specialist clinical pharmacy service provided meets with national standards for the delivery of a pharmacy service.

Main duties of the job

To develop and deliver clinical pharmacy services in CRHT to nationally approved standards in conjunction with the CRHT pharmacists, technician and pharmacy team thereby ensuring safe and appropriate use of medicines in order to maximise benefit and minimise risk. To support the CRHT clinical pharmacy team. To liaise with inpatient wards, community teams and the pharmacy team to ensure smooth transfer of care. To provide cover for other pharmacists when requested to do so. To work within own scope of competence as an Independent non-medical prescriber within CRHTs.

Person Specification
Qualifications
  • Master's degree in Pharmacy
  • Registration as Pharmacist with General Pharmaceutical Council (GPhC)
  • Professionally up to date including Continuing Professional Development (CPD) in line with GPhC requirements
Experience
  • Variety of hospital based pharmacy services
  • Variety of clinical pharmacy experience including mental health
  • Provision of specialist advice about medicines to clinicians in secondary care
  • Analysis and interpretation of queries related to patients and medicines
  • Initiating and implementing change in services
  • Development of medicines related policies and guidelines
  • Supervision of pharmacy staff / technicians
  • Planning and organises medicines related audit
  • Participation in practice based research / NICE guidance implementation
Knowledge
  • Microsoft office: Excel, Word and Outlook for email
  • IT skills including use of databases
  • Developing and implementing Standard Operating Procedures (SOP)
  • Specialist clinical knowledge relating to medicines in mental health
  • Specialist knowledge of medicines to treat physical health
