This is a senior clinical lead nurse role in the South region for Libertatem Healthcare Group, which provides complex care services for children and adults with conditions including brain injuries, spinal injuries, cerebral palsy, learning disabilities, autism, and mental health. The role involves managing a team of clinical leads, overseeing care packages, and providing clinical leadership and governance.

Overview

Senior Clinical Lead Nurse responsible for delivering personalised client-centred service outcomes, carrying out care assessments, writing care plans, and managing competencies. Provides leadership to the Clinical Leads and support staff, and collaborates with Regional Operations Managers to deliver excellent care delivery. Reports to the Head of Clinical Services and Registered Manager, working with Field Care Managers to direct aspects of their work.

Main duties / Responsibilities
  • Delivering personalised client-centred service outcomes, carrying out care assessments, writing care plans, and managing competencies.
  • Providing leadership to the Clinical Leads and support staff, positively supporting and working alongside Regional Operations Managers to deliver excellent care delivery.
  • Reporting into the Head of Clinical Services and Registered Manager.
  • Working with Field Care Managers, directing aspects of their work where needed.
  • Maintaining professional skills and undertaking required training for personal professional growth.
  • Performing in accordance with national standards, legislative requirements, relevant regulations, and best practices.
You’ll have the opportunity to
  • Oversee and manage the Clinical Leads to maintain standards and compliance.
  • Monitor client and family-centred outcomes and guide changes proactively.
  • Support the Head of Clinical Services and the Registered Manager to develop clinical governance and community services.
  • Act as a responsive escalation, providing support and advice to the clinical team within care packages.
  • Develop individualised care plans in partnership with the client, family, and other healthcare professionals.
  • Undertake initial holistic nursing assessments with the client, family, carers, and where necessary, other providers.
  • Support the region by visiting clients with complex needs and observe practice of Clinical Leads on visits.
  • Engage with Regional Operations Managers to ensure a holistic approach centered on the client.
  • Ensure high standards of recording and documentation.
  • Motivate and manage the nursing/care team, embedding company values and a culture of pride.
  • Support field-based care staff through regular supervisions, competency assessments, and appraisals.
  • Report against targets and standards; implement action plans for service improvements.
  • Maintain compliance with statutory, mandatory, and contractual requirements.
Working with Others
  • Collaborate with Central Support (including HR) to motivate staff through supervision and appraisals.
  • Coordinate with Care Coordination on rotas, ensuring staff are trained, assessed, and signed off for competent lone working.
  • Promote welfare of clients and work with multidisciplinary teams to maximise opportunities for clients.
  • Share information, concerns, and improvement ideas with Libertatem’s senior management.
Other duties
  • Seek opportunities for personal and professional growth and development.
  • Adhere to the NMC Code of Conduct and maintain up-to-date best practice and evidence-based research.
  • Attend statutory training and revalidation as required; maintain regulatory awareness of Libertatem’s service.
  • Be part of the out-of-hours escalation on-call rota.
A Bit About You

This role blends clinical nursing skills with coaching and management of people and services. Experience delivering complex care in the community is essential. Experience with spinal injury, acquired brain injuries, cerebral palsy, learning disabilities, autism, and mental health is desirable. Must be able to pass knowledge to others and work with both adults and paediatric care; training will be provided where required.

  • Ability to develop effective working relationships with key stakeholders such as Case Managers, Integrated Care Systems, General Practitioners, Consultants, and other healthcare professionals
