Job description
Overview

RBH's surgical programme makes it the largest cardiothoracic unit in the UK. We are looking for a Senior Clinical Fellow in Thoracic Surgery to assist consultant staff in preoperative assessment, operative and postoperative management, and to undertake administrative duties including staffing clinics, arranging operation lists, and overseeing preoperative and postoperative care. Regular ward rounds and out-of-hours clinical cover are expected. This post may close earlier if a sufficient number of applications have been received. Candidate must have 3 years of pure thoracic experience.

Main duties

Aims and responsibilities of the post: To coordinate and supervise the admission and discharge of patients from the unit; maintain accurate notes; up-to-date cardiac and thoracic monthly audits; and record patient details in the database. Take charge of all patients in the unit under the direction of the attached consultant surgeon. Coordinate and supervise the work of junior medical staff. Operative duties are supervised with targets in operative experience as prospectively set; senior post holders may be assigned to one consultant but may support other firms as needed. Junior post holders rotate between consultant firms and engage in a range of cardiothoracic work. Provide immediate cover to patients in recovery and on the general cardiothoracic ward and cover intensive care emergencies. Consultant cover will be available at all times.

Job responsibilities

Clinical: Attend and assist in outpatient clinics; ensure adequate staffing and efficient clinic operation; dictate clinic letters promptly. Attend consultant ward rounds; conduct formal ward rounds daily including weekends/holidays as part of on-call responsibilities; rounds should be used as teaching opportunities. Out-of-hours: night-time supervision of surgical patients, including those in ICU/Recovery; communicate major problems to the responsible consultant. Weekend on-call: ward rounds, emergency operations and assessment of in-patients or outside referrals. Advise on discharge of patients under the cardiothoracic surgical consultants.

Theatre: Ensure theatre cases are appropriately staffed; involve junior staff; assign junior staff to cases; ensure junior assistants are available to assist; ensure prompt transfer of patients; maintain communication with relatives; ensure data entry at conclusion of operations; operator to create accurate operation notes; if operations are delayed or cancelled, inform wards and patients.

Admission: Coordinate theatre schedules with consultants; maximize efficient bed use; participate in weekly planning meetings. Waiting list management: maintain knowledge of active waiting lists and participate in equitable admission decisions. The post holder may perform duties in emergencies; such commitments are exceptional and not regular.

Qualifications and experience

Essential

  • Full GMC registration
  • Entry on the GMC specialist register (or within six months by the date of interview)
  • FRCS CTh or equivalent qualification

Clinical experience, knowledge and skills

  • Comprehensive experience in both elective and emergency thoracic surgery
  • 3 Years of pure thoracic experience
  • Previous appointments in recognised high-quality thoracic surgery units
  • Proven competency as an independent operator in techniques such as VATS lobectomy, mesothelioma surgery and emergency airway management using bronchoscopy and laser

Desirable

  • Credentialed training in robotic thoracic resections

Research and audit

  • PhD or equivalent higher degree relevant to cardiothoracic surgery
  • Understanding of clinical audit, research and medical statistics
  • Experience in academic supervision and leadership

Desirable

  • Evidence of audit activity/publications in thoracic surgery topics
