A healthcare service provider in Grimsby is seeking a Senior Care Worker to provide compassionate health and social care to palliative patients and families in their homes. The role involves day-to-day delivery of care, managing case referrals, and requires experience in palliative care and a Level 3 qualification in Health and Social Care. Flexible working patterns are available including evenings and weekends.
We are looking for Senior Care Workers to join our service. The Haven team provides Health & Social Care services to palliative/end of life care patients and families in their own homes. The service provides 24/7 health and social care support for people who are:
The service is a support service working in conjunction with the community nursing teams, however the service will provide registered nursing support and all patients referred to the service will be case managed by a registered nurse.
The team will provide a mix of health and social care to the patient and their family to enable people to meet their preferred priorities of care and preferred place of death.
The service will work through a central point of referral and care support will be agreed by the support at home co-ordinator.

Rota pattern: You will be required to work a rota pattern which includes weekends and Bank Holidays. Shifts are a combination of 8am-1pm and 5-10pm (split), and 8am-4.30pm and includes weekends and bank holidays
Salary Range 3£24,968 per annum - fixed salary point (pay award pending)
The Haven team provides Health & Social Care services to palliative/end of life care patients and families in their own homes. Day services are available between 8am-10pm and night care 10pm -8am.
You will be responsible for the day to day delivery of nursing & social care needs, support and respite care to patients and their families in the community.
A Level 3 qualification in Health and Social care (or willing to complete) is essential along with a minimum of one year recent experience of caring for people and families. Experience of working within a palliative or community setting preferred.
Effective communication/customer care skills are essential along with a caring, compassionate manner and the ability to work within a team. Knowledge and understanding of the needs and issues of patients and families with palliative/end of life conditions is desirable.
It is essential that you have the ability to transport yourself and equipment across locations within North East Lincolnshire, to deadlines. Pool cars are available to use for this.
