A local healthcare practice in Madeley is seeking a forward-thinking GP to work 4-8 sessions per week. We are seeking a forward thinking, motivated GP to work between 4 - 8 sessions per week. We are a well run, welcoming, family orientated practice in the south of Telford close to Ironbridge and the Shropshire countryside. We are a steadily growing practice of 8500 patients which moved into a purpose built surgery in 2015.
Our belief is centred around continuity of care, good access and support for our employees. We are in the top two in Shropshire for good access and pride ourselves on doing what is best for patients.
We are looking for someone for between four and eight sessions, Monday to Friday with flexibility offered around days and hours.
Usual duties of a GP including telephone, e-consults and surgery consultations, home visits, prescriptions, dealing with correspondence and queries in a timely manner. On-call shifts are on a rota basis and you will benefit from a highly supportive working environment.
We are a friendly team consisting of 3 GP partners, 1 Salaried GP, a Business Development Partner and an excellent nursing and healthcare team.
We have two Practice nurses, two health care assistants and, through our PCN, a Clinical Pharmacist, Pharmacy Assistant, onsite physio and comprehensive social prescribing team. We have a well trained administrative team led by our practice manager. Our Business Partner is also a director of our PCN ensuring the practices interests are well represented.
We have Telford Mind - the local affiliate of the national charity Mind - based in our practice enabling seamless care for patient with mental health issues. Our team of social prescribers covers all ages from Children and Young People to frailty.
JOB TITLE: SALARIED GENERAL PRACTITIONER
REPORTS TO: THE PRACTICE MANAGER
HOURS: 4-8 Sessions per week, happy to consider part-time/ flexible.
Job summary:
The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
Clinical responsibilities:
In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs of illness
Developing care plans for health in consultation with patients and in line with current practice disease management protocols
Providing counselling and health education
Admitting or discharging patients to and from theeload and referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes to agreed standards
Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
