A dedicated healthcare provider in Hailsham seeks an enthusiastic Physical Health Practitioner. The role involves supporting interventions in physical health care, conducting assessments, and engaging in health promotion activities. To be an autonomous practitioner supporting delivery of interventions in physical health care.
Supporting patients with long term heath conditions maintaining accurate and relevant data
To undertake physical health assessments , attendance of Recovery Reviews when required
To work with colleagues to undertake a range of health promotion activities e.g. healthy lifestyle checks, sexual health advice, weight management programmes, and signposting to appropriate services/agencies.
Responsible for the maintenance of stock control for medical and surgical equipment and consumables as appropriate.
As physical health Practitioner, you will be supporting the delivery of implementing bespoke intervention which support our patients with any given physical issues .
Your role will be diverse, work is varied, can be fast paced and demanding, so the post holder must be flexible in their practices.

You will be line managed by the modern matron but will build positive working relationships with a MDT who comprises of consultant psychiatrist, nurses , HCA, to other professional such a psychology , social workers to our security reception team.
