Qualifications

• Minimum of 12 months experience supporting individuals with learning disabilities or autism.
• Good health and ability to perform all care tasks.

Responsibilities

• Support service users in transitioning from various settings and establishing positive lifestyles.
• Assist clients in maintaining tenancies, participating in the community and developing independent living skills.
• Help clients achieve personal goals and foster a fulfilling life.
• Act as a Key Worker, supporting clients' aspirations and planning their support priorities.
• Work collaboratively with Care Managers, health professionals and families to deliver person-centred care.
• Provide support during evening, weekend and bank holiday shifts as required.
