Job description

The Integrated Care Homes Service is looking for Registered Nurses with community nursing experience, or a student nurse who has completed their management placement in the community, to join our developing team.

As a Registered Nurse, you will provide nursing interventions to a wide range of patients, by in reaching into Nottingham City Care Homes to deliver NHS services. The services overarching aim is to ensure that care home residents and staff have an improved experience through high quality care and access to familiar health professionals with the skills to meet their needs. A single point of access enables the care home staff to refer patients directly to the service enabling the prioritisation and allocation of referrals.

The successful candidates will require up to date clinical knowledge and skills and will be required to work autonomously, using initiative to deliver nursing care within a supportive team environment. They will be working with a diverse population providing support and nursing care to people who have complex health problems, with the aim to maintain and maximise their independence and prevent avoidable hospital admissions. You will be supported to learn and develop the skills required for this role.

Main duties of the job

The role of the Registered Nurse is to support the District Nurse in providing nursing interventions to patients whose home is within a residential care home. This includes insulin administration, wound care, continence care, palliative support and EOL care.

As a Registered Nurse within this integrated service facilitates the development of new skills and knowledge across all the elements of the service to enable a holistic approach to patient care, improving the patient outcomes.

Job responsibilities

Job Purpose

To provide community nursing care to a wide variety of patients in residential homes across Nottingham City.

To work within the wider integrated care homes service providing holistic care to patients. This nursing care includes acute and chronic wound management, medication administration, catheter care, palliative care, long term conditions/ self-care.

To provide holistic and individualised care to patients and carers as delegated by the Team Leader or deputy, working autonomously and as part of a team.

To regularly take responsibility for care and workload management in the absence of the Team Leader or deputy with named support from another team.

Dimensions

To provide nursing care across a diverse population for patients who are living within a care home and are registered with a Nottingham City GP.

To participate actively in the services training strategy, including mandatory training and other identified necessary for this role.

To work across the Nottingham City Integrated Care Homes and Community Nursing Teams.

To supervise and support student nurses on placements with the community nursing teams.

To supervise unregistered nurses who have a delegated caseload.

Establish and maintain communication with individuals and groups in a wide variety of settings and situations in the Integrated Care Homes service and wider CityCare services.

Liaise and interact with other agencies and disciplines

To be responsible for the communication and effective feedback to the team leader or deputy.

Support effective team working both in the team and within the care homes.

To be responsible for the development of own knowledge and clinical practice and recognises one's own limitations.

Act as a mentor to a variety of nursing students.

Participate in the development of other team members.

Participate in the supervision framework as a supervisee and supervisor.

Contribute to the implementation of service developments.

Participate in the Personal Development process and take responsibility for reaching agreed development objectives.

Attend mandatory training.

Utilise enhanced knowledge and skills to promote continuous service improvements.

Promote and maintain best practice that the health safety and security of self, patients and colleagues in accordance with policies procedures and guidelines of NCCP.

Ensure that confidentiality is protected at all times.

Practice in accordance with procedures and guidelines for the Protection of Vulnerable Adults and Child Protection.

In the absence of District Nurse Team Leader or deputy takes responsibility for the needs of patients and carers on the community nursing caseload and ensures that appropriate delegation and supervision of work is carried out in accordance to the skills and abilities of team member.

Qualifications
  • RN qualification on part 1 or part 2 of the register
Experience
