Job Description

Role Summary

We are seeking only the most capable, fully qualified Registered Nurses possessing postgraduate nursing qualifications and a minimum of two years’ substantive, post-registration clinical experience. This requirement is central to our innovative and high-standard approach to home-based care for frail elderly clients. Our mission is to disrupt and transform the home care model—raising standards, supporting earlier discharge from hospital, and proactively mitigating/reducing rates of unplanned hospital readmission through expert clinical care and advanced care planning.

Core Responsibilities
  • Deliver superior, evidence-based nursing care to frail elderly clients with multiple comorbidities, managing complex conditions such as diabetes, cardiovascular disease, COPD, dementia, and frailty syndromes.
  • Undertake comprehensive assessments, develop, implement, and review individualised care plans incorporating best clinical practice and client/family input.
  • Exercise advanced clinical judgement to manage polypharmacy safely, monitor drug interactions, and coordinate with GPs/pharmacists to optimise medication regimes.
  • Perform high-level nursing interventions: wound management, catheter care, diabetes care, nutritional assessments, and advanced end-of-life care where appropriate.
  • Facilitate early discharge from secondary/acute care and actively contribute to strategies that reduce readmission—delivering continuity and preventing crisis escalation.
  • Champion EdenCare’s drive for sector disruption by delivering care that not only meets but exceeds CQC regulatory and quality requirements.
  • Maintain rigorous, audit-ready clinical documentation and safeguard compliance at all times.
  • Mentor, supervise, and train less-experienced colleagues, championing continuous service improvement.
  • Respond decisively to acute changes in clinical status, ensuring seamless escalation and safeguarding.
Skills & Requirements
  • Current NMC registration as a Registered Nurse (Adult / Mental Health / Community).
  • Mandatory postgraduate nursing qualification (PGDip, MSc, or equivalent advanced certificate).
  • Minimum two years of substantive, post-qualification clinical experience in elderly care, community, or acute healthcare settings.
  • Proven track record in managing frail, complex patients and expert, safe polypharmacy.
  • Strong assessment, care planning, and clinical risk management skills.
  • Evidence of commitment to innovation and quality improvement in care.
  • Outstanding communication, demonstrable empathy, and a commitment to EdenCare’s transformational vision for home care.
