Job description

To work as a key member of a multi-disciplinary team by offering information, advocacy and support to substance misusers in line with their recovery action plan in order to improve the service user experience and promote recovery.

Main duties of the job

To undertake needs led assessments of people with substance misuse problems that require additional support to address their social needs.

To inform recovery action plans in consultation with service users and their case worker that address their social care needs.

To implement, monitor and review a range of interventions in relation to social needs.

To support and motivate service users to attend and take part in appointments/activities which promote their recovery.

To advocate for service users where appropriate to promote recovery.

To work in partnership with other professionals/agencies to ensure that service users\' needs are met.

To support service users to engage with mutual aid/self-help support groups and services to help them in their recovery journey and promote independence.

To involve families and carers where appropriate.

To encourage and support users to adopt safer practices and lead healthier lifestyles.

To actively promote service user and carer involvement in the planning and delivery of services.

To actively participate in team meetings.

Job responsibilities

Education/Qualifications
  • Educated to NVQ level 3 in Health and Social care (or an equivalent qualification in Health and Social care)
Experience
  • Demonstrable experience of working with clients in a health or social care setting. Experience of providing support to vulnerable/socially excluded individuals/groups. Experience of working as part of a team to achieve a shared vision/goal. Experience of maintaining quality standards in one\'s work
  • Awareness of equal opportunities legislation An awareness of policies & procedures in relation to confidentiality, information sharing, and safeguarding
Skills & Abilities
  • Ability to handle challenging situations and behave in a professional manner. Ability to use initiative but to consult with others when necessary. Ability to communicate with a wide range of people including service users, carers, members of the public and other professionals.
