Overview

Upton Surgery is a friendly, forward thinking General Practice with onsite NHS Community staff and Social Care staff. We have a holistic, integrated care approach for our 12,200 patients. We are a training practice with 5 GP partners. Located just 10 minutes from junction 1 of the M50, 15 minutes from Worcester and Malvern. 10 minutes from Tewkesbury and 30 minutes from Cheltenham.

We are seeking to employ a motivated Receptionist / Care Navigator with exceptional customer focus to support our developing triage and care navigation roles as part of our busy Reception Team.

Responsibilities
  • To provide a consistent point of contact for patients and staff, enabling patients to get the right coordinated support at the right time to manage a wide range of needs.
  • To work alongside our GPs assisting in triaging patients and providing active signposting to a range of statutory and voluntary sector services.
  • To provide first-class reception and clerical support and to support triage and care navigation, ensuring patients are directed to the most appropriate services.
  • To use IT systems daily (Emis Web, Docman, Accurx, Abtrace, Microsoft & X-on).
  • To work collaboratively with the clinical and administrative teams.
