Job description

Camden Rapid Access Services (RAS) is a Physical Health admission avoidance and supported early discharge service with three pathways – Discharge to Assess, Urgent Community Response and Virtual Ward. The role works across all three pathways within Camden RAS.

Applicants should ideally possess a valid UK Driving Licence. Non‑driving candidates will be provided an Oyster card while on shift and must undertake the same number of visits per day as driving colleagues.

We are seeking a Band 6 Registered Nurse or Paramedic with substantial acute care experience. An ideal candidate is confident in top‑to‑toe examinations and succinct history taking.

Our aim is to avoid unnecessary hospital admissions, supporting patients to stay in their own homes and improving independence.

Responsibilities
  • Conduct physical assessments and take a thorough, succinct history focusing on the presenting complaint.
  • Prioritise patients based on clinical need and coordinate care across multidisciplinary teams.
  • Maintain accurate, timely, and legible clinical records (electronic and handwritten) in line with CNWL policies.
  • Act as Clinical Supervisor for Health‑Care Assistants and/or Band 5 nurses/paramedics.
  • Serve as Nurse / Paramedic in Charge of Shift (Clinician of the Day).
  • Apply CNWL behaviour core values to all aspects of work.
Shift Pattern
  • Long day: 09:00 to 22:00 (occasionally 08:00 to 21:00).
  • Early shift: 09:00 to 19:00.
  • Late shift: 12:00 to 22:00.

Lone working is common, including evening hours. Robust safety systems are in place and the trust maintains an excellent safety record.

Person Specification
Education and Qualifications
  • Registered Nurse with current NMC registration OR Paramedic with current HCPC registration.
  • Bachelor’s degree (or working towards).
  • Evidence of continued professional development.
  • Advanced Clinical Assessment Course (or willingness to undertake).
Experience
  • Substantial acute care experience – relevant post‑registration experience in A&E, UCC, ITU, AMU, MAU, CCU or A&E.
  • Evidence of working within the multidisciplinary team.
  • Understanding of primary care role in admission prevention and timely discharge.
Skills and Knowledge
  • Maintaining accurate, timely, concise, and legible clinical records (electronic and handwritten).
  • Strong communication skills – able to discuss sensitive issues supportively and challenge assertively when required.
  • Proficient in SystmOne and Microsoft Word.
  • Knowledge of the Mental Health Act.
  • Proficiency in safeguarding and advocacy.
