Conduct comprehensive mental health assessments for patients presenting to the Acute Trust with suspected or actual mental health issues.
Provide specialist advice and support to acute hospital staff to manage mental health presentations effectively.
Work collaboratively with a range of professionals across health and social care to ensure safe and effective discharge planning and admission avoidance.
Participate in the PLT development programme, including training and supervision to support progression to Band 6. Work rotational shifts, including evenings and weekends, and be flexible to support service needs across the Trust.
Maintain accurate clinical records and contribute to audit and service evaluation activities.
