Healthy Family Support Practitioner – Full-Time Vacancy

Working with children and young people 5‑9 years old and their families: Basildon Area.

We are seeking a passionate, dynamic and experienced 5+ years practitioner to join our Healthy Family Team. Our teams are fully integrated, consisting of Health Visitors, School Nurses, Support Practitioners, Family Hub Practitioners and Community Connectors.

Role Overview

5+ Healthy Family Support Practitioner Role

Responsibilities
  • Work with children, young people and their families on a one‑to‑one or group basis, using a variety of interventions to identify needs and achieve positive outcomes.
  • Partner with parents/carers to empower them and enable better outcomes for their children.
  • Co‑operate with children’s services, health agencies, primary schools and other community‑based services to provide the most effective service for children, young people and families.
  • Undertake developmental and health reviews of children and young people at key stages of life (0‑5 years on occasions).
  • Support the delivery of the Healthy Child Programme in an integrated team.
  • Promote first relationships and attachment.
  • Promote the Baby Friendly Initiative (BFI).
  • Provide universal and targeted support to families through the delivery of group sessions.
  • Deliver evidence‑based parenting support to all families.
  • Provide evidence‑based interventions with families to support children becoming school‑ready and meeting age‑appropriate levels of development.
  • Organise and undertake integrated two‑year reviews in partnership with families and the child’s early years setting.
  • Participate in and deliver ECFWS virtual sessions to ensure all families are reached and have meaningful contact and achievable outcomes.
  • Make contact with families where parental conflict has been identified as a risk factor, providing brief intervention support and advice.
  • Participate and lead on health promotion activities in family hubs, delivery sites, external venues and events.
  • Provide coaching and skills training to young people and their parents to support emotional literacy and overall wellbeing.
  • Deliver educational sessions to parents, children and young people, adapting style and content to meet the audience.
  • Work with specialist providers to provide dedicated support to families of children aged 6‑19, minimising harm and building resilience.
  • Develop self‑confidence, self‑esteem and emotional wellbeing within families in an empowering way.
  • Support the development of Health Family hubs and delivery sites as sanctuaries for families fleeing domestic abuse.
Essential Requirements
  • Minimum NVQ Level 3 qualification and experience working with children within school, home and community settings.
  • Confidence when delivering advice and support to parents and carers.
  • Good understanding of accurate recording.
  • Sound knowledge of GDPR.
  • Experience and empathy when building relationships with vulnerable children and adults.
  • Ability to drive and access to a vehicle.

This role is based in the South Essex locality, working in and around the Basildon Area.

As per NHS guidance this role may require successful applicants to evidence immunisations or immunity for MMR and Chicken Pox, due to the clinical focus of some tasks and the requirement to work with vulnerable families, young people and children.

The role may also require successful applicants to wear PPE, including a protective mask, and to follow social distancing rules as highlighted in NHS guidelines. Successful applicants will be required to remain compliant with any future mandatory requirements introduced by the Government/NHS.
