Salary Range: $12.00 - $12.00 Hourly | Position Type: Full Time | Job Shift: Mid-Day

Program Instructor – Day Services

Summary – A Program Instructor provides day support services necessary to support persons with disabilities, including transportation by minibus, van, or lightweight truck. The role requires transportation training and knowledge of Activities of Daily Living (ADLs).

Responsibilities
  • Provide direct daily care and assist with ADLs and recreational activities.
  • Plan and execute daily activities with Program Leader and managers.
  • Accompany and transport individuals to community outings and recreational activities.
  • Follow Individual Support Plan (ISP) and document progress toward developmental goals.
  • Transfer individuals into and out of wheelchairs and motor vehicles; use Hoyer Lift as needed.
  • Drive vehicle to social services, rehabilitation centers, training locations, job sites, or other destinations per schedule.
  • Maintain a safe driving record and comply with all local traffic regulations.
  • Keep accurate daily documentation of activities, medications, ISP tracking, and other required records.
  • Adhere to emergency preparedness and response procedures; report incidents immediately per policy.
  • Observe and report changes that could affect safety, including environmental changes, condition changes, or care complaints to Lead Staff, Program Manager, and/or Director of Community Life.
  • Respect each individual’s rights, maintain confidentiality, encourage decision‑making, promote self‑esteem, and protect individuals from abuse and neglect.
  • Communicate professionally with families, service coordinators, and InCommunity employees.
  • Model effective adult behavior for the people served.
  • Maintain supervision of individuals unless specified in the Plan of Care.
  • Consult with staff on progress and problems.
  • Remain on duty for assigned hours and implement the planned schedule.
  • Obtain 16 hours of task‑focused training related to the job assignment.
  • Perform all other duties as assigned.
