Qualifications

• Experience working in primary care or community setting.
• Competent in Long Term Condition management, especially Asthma and Diabetes.
• Experience in delivering childhood and adult immunisations.

Responsibilities

• Conduct structured reviews for patients with asthma and diabetes.
• Manage and administer immunisations and vaccinations for children and adults.
• Ensure confidentiality is maintained at all times.

Registered Nurse with current NMC registration
