Overview

We have an exciting opportunity to recruit an Intellectual disability nurse to our Primary care liaison Team. We are looking for a motivated and enthusiastic RNLD who is passionate about supporting reasonable adjustments for patients with intellectual disabilities in the community. We welcome applicants who are interested in supporting people with intellectual disability to enable them to have their physical health needs met within a primary care setting. We support practices to keep a learning disability register and promote the health needs of the population. We are looking for someone who is passionate about enabling our patients to access generic health screening and break down barriers in order to achieve this. Responsibilities
  • Work with individuals to facilitate their access to primary health care services and liaise with other health professionals within multi agency teams to advocate for service users.
  • Work with local GP Practices to help identify patients who are eligible for an annual health check are flagged and are encouraged to have an annual health check.
  • Deliver training around the Enhanced Service around health checks to primary care colleagues.
  • Work with the national screening programmes to support them to meet the needs of the local intellectual disability population around breaking down barriers to support access to screening.
  • The post holder will be committed to improving the life chances of people with intellectual disabilities.
  • The post holder\`s key role is to support adults with intellectual disabilities to access mainstream health services.
  • The post is within Newark & Sherwood locality within Nottinghamshire.
  • The post holder will participate in the delivery of enhanced training to primary care colleagues around the annual health check and reasonable adjustments for people with intellectual disabilities.
  • Ensure those aged 14+ who meet the eligibility are included on the annual health check register.
