A medical practice in Rochdale is seeking a motivated Clinical Pharmacist. This role involves conducting medication reviews and managing prescriptions to support patients with long-term conditions. The pharmacist will also provide pharmaceutical support to improve patient care and work on quality improvement projects. Essential qualifications include registration with the General Pharmaceutical Council and a relevant clinical diploma. We are looking to recruit a motivated, committed and enthusiastic Clinical Pharmacist with previous experience of working in General Practice to join our friendly practice with a patient population of around 4600.
Clinical staff include two GP's , Practice Nurse, Advance Nurse Practitioner and health care Assistant
The role will involve working with the lead GP and Medicinesmanagement team for various quality improvement projects.
The role will have patient facing element either face to face or telephone - repeat prescription management and structured medication reviews.
This will also include the management and review of patients with poly-pharmacy.
The post holder will work within their clinical competencies to provide specialist pharmaceutical support to all our patients and will be working with members of the wider healthcare team to improve care and support safe and effective prescribing.
The post holder will need to be proactively involved in quality and improvement and audit as well as managing aspects of the Quality and Outcomes Framework
The Practice is located in Central Rochdale with a tram stop in front of the building and Rochdale train Station to the side.
We are housed on the 2nd Floor of a Modern Lift Building.
We are part of the Canalside Network.
