A healthcare provider in Lydney is seeking a dedicated practice nurse for various consultations and long-term condition management. The role requires a registered nurse with strong communication and IT skills. You will provide high-standard patient care and contribute to quality improvement initiatives. The position is part-time, requiring 20-28 hours per week with flexibility in working patterns. An exciting opportunity has come up to join the growing practice nurse team at GDOC LTD. Developing long term condition management & support, while providing an excellent standard of care to our patients. The main practice of work will be Lydney Practice.
Hours: 20-28 hours per week

Main duties of the jobTo provide a high standard of nursing care for patients, as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced RGN and any additional roles agreed between the nurse and practice management team as appropriate, having regard to the nurses training and competencies, alongside a specialist qualification in a long term condition. Then managing that workload alongside the wider practice team.
Working always within your professional competencies, you will make yourself available to undertake a variety of duties including surgery consultations and emergencies; telephone consultations and queries; triaging; and dealing with queries, patient test results, patient medical reports and examinations; and NHS/private paperwork and correspondence.

Duties Specific to Role
Primary care nurse duties including (but not limited to) face to face consultations, video consultations, telephone consultations, e-consultations and queries. Clinical administration, and dealing with queries, paperwork and correspondence in a timely fashion.
Provision of care for acute and long-term conditions (physical and mental), and contributing to the delivery of QOF and enhanced services.
Provision of preventative care, including health promotion advice, vaccinations and screening, alongside a specialist qualification at degree level in a long term condition.
Support of patients in the developing self-management strategies and in fostering patient engagement with the Practice in the specific long term condition.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Recording clear and contemporaneous I.T. based consultation notes to agreed standards;
Collecting data for audit purposes as per Practice protocols.
Reviewing patients in the interests of quality improvement and safety. i.e. MRHA safety alerts and dementia reviews.
Prescribing in accordance with locally agreed or national guidelines.
Cover for other practice nurses will be provided by reciprocal sharing of correspondence, results and other administration.
Contributing to patient safety by maintaining good infection control thorough monitoring and maintenance of stock and equipment to include refrigeration, sterilising equipment and emergency equipment.
Complying with the Nursing and Midwifery Councils Code and professional standards for nurses.
Complying with CQC requirements for primary care.
Complying with the training requirements of G DOC and of the NMC
Complying with requirements for revalidation
Working conditions
