A healthcare organization in Cockermouth is seeking an experienced Practice Nurse to support chronic disease management within a primary healthcare team. The ideal candidate will have experience in at least two long-term conditions, with a strong emphasis on maintaining high standards of nursing care. The role is full-time but part-time applications will also be considered. A competitive hourly rate ranging from £15.90 to £19.31 is offered, depending on qualifications and experience.

The successful candidate/s would undertake the role as a Practice Nurse focusing on chronic disease. Although this is a full time role, we would consider part time applicants.
Towork as part of the primary health care team in providing and maintaining ahigh standards of nursing care to the practice population and work within thescope of the NMC code of professional conduct.

JOB DESCRIPTION DUTIES & RESPONSIBILITIES
Clinical
Undertake the general duties of a Practice Nurse focusing on Chronic Disease
Be competent to be able to offer chronic disease management to support the practice in achieving quality indicators and targets of QOF, LIS and DES including COPD/Asthma/Heart Failure/Diabetes
Carry out childhood immunisations
Hold a non-medical prescribing qualification, or willingness to undertake training to achieve this
Competent to offer travel health, advice and vaccinations with the potential to lead on Yellow Fever management across the practice
General treatment room duties which includes wound management and minor injuries
To offer family planning and contraceptive advice as required
Qualified to undertake cytology screening
To provide appropriate treatment to the practice population in accordance with practice protocols and procedures
Apply infection control measures within the practice according to local and national guidance
Maintain accurate records and document consultations according to local and NMC guidelines
Ensure awareness of statutory and local clinical protection procedures, including systems of referral including child protection strategies
To work in accordance with PGDs and support HCAs working to PSDs
To liaise with other members of the primary health care team as required
General Duties
To ensure accurate patient details and consultations are recorded accurately on EMIS
To Chaperone for the GPs if requested to do so
Actively participate in nursing meetings, following up actions as required
Responsibility for standard operating procedures (SOPs) and policies within own area of work and ensuring these are kept up to date
Liaising with other team members regarding changes to service
Ensuring annual leave is covered with other staff members in advance and informing the Practice Manager of any staff shortages
Educational
Participate in continuing education as identified whether personally or by the Practice to maintain a high level of professional knowledge and skills
Promote the continuing professional development of yourself and other nurses and healthcare assistants, facilitating a learning culture within the team
Offer innovative ways of working and opportunities to facilitate learning

Attendance and completion of mandatory training as directed by the practice
Adhere to practice policies and procedures
Health & Safety
Practice staff will comply with all policies in relation to Health and safety and ensure they complete any mandatory training as advised by the Practice Manager.
Maintain an up to date knowledge of health and safety and infection control statutory and best practice guidelines and ensure implementation across the business.
Information Governance
Practice staff must keep up-to-date with the requirements of information governance and must follow practice policies and procedures to ensure information is dealt with legally, securely, efficiently and effectively. Staff must appropriately manage the records they create or hold during the course of their employment with the practice, making the records available for sharing in a controlled manner, subject to statutory requirements and agreed security and confidentiality policies, procedures and guidelines (e.g. Freedom of Information Act 2000, Caldicott guidelines). If you are required to make entries into health records, you must ensure these entries are legible and attributable, and that record keeping is contemporaneous.
