Salary negotiable based on experience and skillset. Hours: 37.5 Full Time (Job Share will be considered).

Job Summary

East Durham Medical Group is seeking an enthusiastic, forward‑thinking experienced Practice Nurse to join our highly committed primary health care team. We are a teaching and training SystmOne Practice with over 18,500 patients.

We provide a 6‑day a week service covering 6‑8pm and weekends on a rotating basis.

The ideal candidate will be based at two of our six sites and will be expected to travel to other sites as needed.

Responsibilities
  • Develop, implement and embed health promotion and well‑being programmes.
  • Support the clinical team in the development of individual treatment plans for chronic disease patients.
  • Identify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects on the patients’ health.
  • Provide routine nursing care to patients as required in accordance with clinical evidence, NICE and the NHS.
  • Provide wound care (ulcer/Doppler etc.) to patients.
  • Provide travel medicine services.
  • Refer patients directly to other services as appropriate.
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
  • Maintain accurate clinical records in conjunction with extant legislation.
Person Specification
Essential
  • Practical experience in the management of long‑term conditions, particularly Asthma, COPD, and Diabetes.
Desirable
  • Experience of working in general practice.
