Job description

We are seeking a Practice Nurse to join our friendly team.

Previous GP Practice and chronic disease management experience is desirable although not essential as training is available.

Emis Web clinical system experience would be an advantage.

Salary depending on experience.

Main duties of the job

To deliver high quality integrated nursing care to the practice population and promote health and well being within the practice population

To work with clinical and non-clinical practice staff and other members of the wider healthcare team contributing to the achievement of QoF targets

Job responsibilities
  • To provide assessment, treatment, screening and health education services and advice
  • Provide nursing service/treatments to patients independently or in participation with general practitioners with reference to agreed protocols and guidelines.
  • To work on own initiative whilst recognising when referral to other health professionals is needed.
  • To be familiar with changes and progress in nursing care.

Nursing procedures to include, but not limited to -

  • Measuring blood pressure
  • Routine injections as prescribed
  • Cervical Smears
  • Childhood and Adult immunisations, including reading and reviewing Patient Group Directives (PGDs) and Patient Specific Directives (PSDs)
  • Well Person Health Checks including Foreign Travel Counselling, NHS Health Checks, Lifestyle and New Patient Checks
  • ECGs
  • Ear microsuction and assessment of ears
  • Obesity and weight/diet management
  • Family Planning including assisting in Implants & Coil fittings & Sexual health
  • Chronic disease management and prevention
  • CVD risk assessments and Q risk interpretation and assessments.
  • Occasional home visits for housebound patients.
  • Referrals Podiatry, Counselling, Audiology, etc
  • Ensuring all specimens are checked and sent to laboratory as required
  • To be fully conversant in QOF, read coding and uphold strategies to provide a holistic combined healthcare for the patient.
  • To be responsible for the efficient running of the treatment room and equipment and to ensure the clinical areas are at all times safe for staff and patients.
  • To adhere to strict Infection Control in line with guidelines and policies.
  • Advise patients and relatives/carers on general health care and ailments with referral to GPs as necessary.
