A healthcare practice in Sunbury-on-Thames is seeking a Practice Nurse to join their friendly team. The role includes high-quality integrated nursing care and support for chronic disease management, with training provided if needed. Candidates should have a Nursing Degree, excellent communication skills, and some experience in primary care or GP practices. Joining a multidisciplinary team, you will promote health and well-being for the practice population while delivering various clinical procedures.
We are seeking a Practice Nurse to join our friendly team.
Previous GP Practice and chronic disease management experience is desirable although not essential as training is available.
Emis Web clinical system experience would be an advantage.
Salary depending on experience.
To deliver high quality integrated nursing care to the practice population and promote health and well being within the practice population
To work with clinical and non-clinical practice staff and other members of the wider healthcare team contributing to the achievement of QoF targets
