A local GP practice in Portsmouth is seeking an experienced Practice Nurse to deliver high-quality general practice nursing to their diverse patient population. The ideal candidate will focus on evidence-based care for long-term conditions and preventative health. Responsibilities include managing health promotion strategies and collaborating within a multi-disciplinary team. Previous experience in Primary Care is preferred. The post holderis responsible for the delivery of general practice nursing to the wholepractice population. The focus of the role is both the delivery ofevidence-based practice for patients presenting with a long term condition, andthe provision of preventative health care to the practice population. As anautonomous practitioner the nurse is responsible for the care delivered,demonstrating critical thinking and skills in clinical decision-making in themanagement of patients. They will work collaboratively within the generalpractice team to meet the needs of patients, support the delivery of policy andprocedures, and provide nurse leadership as required.
Assess, plan,implement and evaluate individual treatment plans for patients with a knownlong-term condition.
Proactivelyidentify and manage treatment plans for patients at risk of developing along-term condition as appropriate.
Review medicationfor therapeutic effectiveness, appropriate to patient needs and in accordancewith evidence-based practice and national and practice protocols.
Work withpatients in order to support adherence to prescribed treatments.
Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects and interactions.
Prioritise healthproblems and intervene appropriately to assist the patient in complex, urgentor emergency situations, including initiation of effective emergency care.
Support patientsto adopt health promotion strategies that promote patients to live healthily,and apply principles of self-care.
Support andmanage health needs of women presenting for family planning and cervicalcytology consultations.
Recognise and refer patients presenting with mental health needs in accordance with theNSF for Mental Health.
Implement andparticipate in vaccination and immunisation programmes for both adults andchildren.
Advise, supportand administer vaccinations for patients travelling abroad.
Meet the needs ofpatients presenting for opportunistic wound care.
TheTrafalgar Medical Group Practice is a 27,000 patient list,6GP Partner Practice situated in the heart of Southsea. The Practice operates across 2 sites. The Practice is also the lead Practice for the Portsmouth South Coast Primary Care Network.
This is an exciting opportunity and time to join the nursing team in a forward thinking, developing, busy, and diverse GP Practice. This is an established training Practice and we are looking for an experienced, enthusiastic and motivated Practice Nurse who can integrate well and who has skills to offer which can benefit our patients.
Previous experience within Primary Care would be desirable, but we will also consider applications from nurses whose background will also provide suitable experience and qualifications.
Hours are negotiable, but there will be a requirement to cover staff absences when required.
Salary - Competitive, subject to qualifications and experience.
