Job description

An exciting opportunity has arisen for a highly motivated nurse to join our well-established multi-disciplinary team.

The role is advertised as part-time, 25 hours a week, but hours are open to negotiation, St David's are flexible on the working pattern.

The focus of the role is both the delivery of evidence-based practice for patients presenting with a chronic condition, and the provision of preventative health care.

Main duties of the job

The successful candidate will see patients with minor illnesses and assist with chronic disease management managing reviews and recalls.

In addition, they will work collaboratively within the Primary and Community Healthcare Team to meet the needs ofall patients, support the delivery of policy and procedures, and provide nurse leadership as required.

The post holder will contribute to the delivery of high quality nursing care, providing specialist advice and clinicalservices to patients.

Responsible for the assessment, development, implementation and evaluation of patient needs.

They will work clinically alongside members of the Nursing Team and widermulti-disciplinary teams to ensure the provision of effective, efficient, appropriate quality evidence-based care topatients.

To act as an accountable role model and clinical expert for healthcare.

Work in conjunction with other disciplines in the assessment, planning, implementation and evaluation ofhealthcare service provision to ensure a smooth patient pathway through the service.

To collect and input clinical data to clinical systems: to access and utilise all available clinical information to support care planning and patient management, and support others to do the same.

To ensure effective and safe medicines management processes are adhered to.

To identify target groups for vaccination e.g. flu vaccine, to collect data via clinical reporting systems, and prepare reports on uptake percentages.

Job responsibilities

The prospective Practice Nurse will hold a Registered General Nurse qualification and have an active registration with the NMC, have excellent communication skills with an ability to prioritise your workload and be a Team player ready to assist your work colleagues if required.

Experience in chronic disease case management including, asthma, COPD, Diabetes, CHD and INR. Also experience in the following nursing procedures, cervical cytology, childhood immunisations, cardiac monitoring and ECGs, travel immunisations, minor illness, wound care and phlebotomy.

Hours of works can be negotiated between 8.00 - 6.30 pm Monday to Friday and some flexibility is required.

Other Duties:

Working in accordance with the NMC Code of Conduct

Concise, accurate record keeping and documentation with competent utilisation of the Vision software

Maintenance of treatment room supplies with specific reference to maintenance of the cold chain and stock levels of vaccines.

Maintenance and checking of stock equipment such as resuscitation equipment and emergency medication accompanying the equipment

Any other duties as reasonably requested by senior staff, inclusive of GPs and Practice Manager, providing those duties are within the scope of the role and that the individual is trained and capable.
