Beckington Family Practice is looking for a permanent Practice Nurse for 2 - 4 days per week
We are a friendly, well established and growing semi-rural training practice based South of Bath, offering a supportive environment and with an ethos for a high standard of patient care. We are currently looking for a hardworking, reliable enthusiastic and committed nurse, who shares our ethos to join our nursing team to enhance nursing services within the practice.
The ability to work flexibly to cover extra sessions is an express requirement for this role
Ideally you would have Practice Nurse experience, however we would also like to hear from Nurses who are keen to develop their skills in order to move into Practice Nursing.
Experience or a keen interest in chronic disease management would be desirable but not essential.

JOB SUMMARY
The successful candidate will work at Beckington Family Practice toenhance nursing services within the practice.The Practice Nurse will undertake a range of nursing assessments andprovide appropriate care / treatment in conjunction with GPs, and otherPractice Nurses or independently as appropriate for the need, according topractice policy, protocols etc. ChronicDisease management will form part of the role as well as the mentoring of ourother Practice Nurses and Health Care Assistants. Knowledge of the GMS contract, e.g. TheQuality and Outcomes Framework, is a vital element of the post. The PracticeNurse will strive to improve standards of care and be involved in identifyingpractice population needs. Additionallythe successful candidate will be encouraged and supported to undertake anyfurther training required to ensure appropriate role development.
DUTIES AND RESPONSIBILITIES
ProfessionalResponsibilities
Assist GP's to help deliver effective implementation of management of patientswith chronic diseases as detailed by the Department of Health and Practicepolicies.
Contribute to the development of policies / protocols / guidelineswithin the practice.
Aid thePractice team with implementation of new initiatives in the delivery of chronicdisease management services.
Formulatereports as required for the Practice Manager and or Partners
Support theimplementation of evidence-based guidelines in conjunction with the Practice Nursingteam.
Encourage thedevelopment of integrated services within Primary Care to include integratedpractice based teams and multidisciplinary working.
Promote thecontinuing professional development of yourself and other nurses & HCAswithin the practice.
Recognise theopportunities for working in partnership with the Primary Health careteam.
Participate in an annual appraisal.
Leadership Responsibilities
Provide professional and managerial support to other Practice Nurses,Health Care Assistants and review the monitoring of patients within primarycare.
Offer guidance and support to the other Practice Nurses and Health CareAssistants to improve standards of care if needed.
Participate in chronic disease management clinics within the practice ifasked and only after appropriate training, & provide practical assistanceand professional advice to achieve minimum standards of care.
Provide clinical input where required alongside other Practice Nurses,particularly newly appointed Nurses and GPs, to assist in skill development andsharing of best practice.
Delegate to others, with appropriate skills, knowledge and competence.
Encourage nursing practice in line with relevant evidence basedresearch.
Act as a resource to the general public, patientswithin the health care system and their families, carers and various healthcareprofessionals.
Clinical Responsibilities
Work alongside Health Care Assistants to help develop their skills.
Support the practice in achieving quality indicators and targets of theGMS contract and any relevant National Service Frameworks.
Undertakeopportunistic screening when clinically required.
Performvenepuncture and electrocardiography when required - following appropriatetraining if required.
Undertake wound care/management/dressing techniques.
Carry out ear examination.
Perform cervical screening andpromote breast awareness - following appropriate training if required.
Administer adult and paediatricinjections under an individualised prescription or Patient Group Direction,ensuring safe storage, rotation and disposal of vaccines and drugs.
Undertake accredited training inspecific chronic disease management as or if required. e.g. diabetes,respiratory, epilepsy, CHD etc. following appropriate training if required.
Contribute to the maintenance ofthe practice diseases registers.
Provideadvice on lifestyle issues, assist and support patients in changing theirunhealthy behaviours and refer appropriately.
Provide anadult and childhood immunisation service working to Patient Group Directions -following appropriate training if required.
Deliver travel health care,including vaccinations and advice for patients - following appropriate trainingif required.
Recognise the signs of childabuse, family violence, vulnerable adults, substance abuse, addictivebehaviours, suicidal tendencies and is aware of local, referral procedures.
Ensure that current evidencebased approaches to patient care is standard practice
Apply appropriate infectioncontrol measures when collecting and handling of laboratory specimens,disposing of waste materials, dealing with blood and body fluids spillages andreport and treat sharps injuries.
Undertake first aid andmanagement of emergencies, eg burns, haemorrhage, hypo/hyperglycaemicemergencies/asthma attacks.
Undertake annual training anddemonstrate competency in anaphylaxis and resuscitation techniques
Research, Audit and Quality
Encourage thedevelopment of a practice audit strategy and action plan for Primary Care.
Contribute to the provision of equitable, high quality services forpatients in the locality.
Help supportthe integration across primary, secondary and tertiary care services.
Maintain aninterest in national research initiatives anddisseminate appropriateinformation to health professionals if asked to do so.
Ensure allcare is evidence based using up to date validated literature.
Contribute toresearch and development programmes where required to improve clinicalpractice.
Education and Training
Provide professional advice /support to health care professionals withinthe Primary health care team
Following identified training needs, participate in training andfacilitation of other Practice Nurses & Health Care Assistants.
Providementorship and guidance for Health Care Assistants.
Participatein partnership programmes of awareness raising, detection and prevention ofchronic diseases.
Maintain a level of expertise in specific chronic disease management(when appropriate) and other pertinent areas of health care, and seek todevelop new and improved skills.
Advise members of the public on the concepts of health promotion,education, prevention and protection
Seek out newknowledge of nursing and health by reading, enquiring and partaking incontinuous education programme.
Takeresponsibility for personal development and education including in-servicetraining and annual appraisal.
Direct,supervise and help mentor Nurses and Health Care Assistants in the clinical setting.
Activelyparticipate in multidisciplinary education and training.
Monitor and maintain consistencyof professional standards and take appropriate action if standards are not met.
Maintain professionaldevelopment by attending relevant courses and study days to keep abreast ofcurrent knowledge and developments.
Abide by the legal requirementsand statutory rules relating to practice and record and maintain periodicprofessional registration with RCN & NMC
Communication
Maintaincommunication and liaise with other disciplines across primary, secondary andtertiary care services as and when necessary
Contribute toand work effectively within the primary care team
Assist inclinical audit and the setting and monitoring of standards of care.
Advise and support other membersof the team aiming towards common objectives.

REGISTERED HEALTH PROFESSIONAL

Where theappropriate professional organisation details a requirement in relation tosupervision, it is the responsibility of the post holder to ensure compliancewith this requirement. RISKMANAGEMENT
It is astandard element of the role and responsibility of all staff of BeckingtonFamily Practice that they fulfil a proactive role towards the management ofrisk in all of their actions. Thisentails the risk assessment of all situations, the taking of appropriateactions and reporting of all incidents, near misses and hazards.
RECORDSMANAGEMENT
