We are looking for a motivated and experienced Practice Nurse to join our multidisciplinary team and help us provide safe, high-quality care to our patients.

Responsibilities
  • Managing long-term conditions (e.g. diabetes, asthma, COPD) through routine reviews and proactive care, conducting chronic disease clinics delivering patient care and making referrals to specialist / secondary care where required
  • Providing cervical cytology with subsequent result processing
  • Routine nursing care according to clinical based evidence, NICE and NSF guidelines
  • Administer childhood immunisations, travel vaccines and flu jabs
  • Requesting necessary pathology services
  • Provide contraceptive advice and support
  • Contributing to QOF and local incentive schemes
  • Promoting health education, self-management and preventative care
  • Working closely with GPs, HCAs, Nursing Associates, and Pharmacists to deliver joined-up care
  • Ensuring high standards of infection control and clinical documentation
  • Support Quality and Outcomes Framework (QOF) delivery and other enhanced services

Person Specification
Experience
  • Are a registered nurse (RGN) with current NMC registration
  • Have recent experience working in a GP practice or primary care setting
  • Are competent in key areas such as chronic disease management, immunisations, cytology, and wound care
  • Bring a proactive, patient-centred mindset to your work
  • Communicate effectively and thrive in a collaborative, multidisciplinary environment
  • Are keen to contribute to service development and continuous improvement
