Garden Lane Medical Centre is a busy Chester City Centre Practice with a friendly team of 7 doctors, 6 nurses, healthcare assistant, GP Assistant and phlebotomist. There is a large, extremely helpful team of administration staff.

Job Description
Job responsibilities
Summary To act as part of the practice team, providing care and advice to the practice population using an evidence-based approach to provide care and advice. Promote effective treatment and health promotion. Liaise with others to maintain and enhance a culture which promotes effective multidisciplinary team working within the organisation, and across the wider health economy.
Principle Responsibilities
- Maintain a high standard of patient care in accordance with the NMC code of professional conduct, and within competencies.
- Maintain and develop own clinical and professional expertise by participating in clinical supervision, professional development planning within NMC code of conduct and own competency.
- Act as a resource for other members of the multidisciplinary team.
- Promote the effectiveness of the nursing contribution to the multidisciplinary team.
- Work autonomously under the indirect supervision of the nurse manager, liaising with other members of the multidisciplinary team as required.
- Work flexibly to support and maintain the effectiveness of the practice team.
- Use analytical and judgemental skills in the assessment and management of patients attending with a variety of conditions and problems.
Clinical
- Exercise a high degree of clinical autonomy and judgement in the management of patients with long term conditions in accordance with level of competency and scope of practice.
- Undertake a range of clinical procedures which may include dressings, wound management, removal of sutures, blood pressure monitoring, ECG recording, spirometry, immunisation and vaccinations, phlebotomy, cervical cytology, ear syringing, basic life support, nebulisation and other procedures within competency and under supervision.
- Provide health promotion and advice, screen patients for risk factors and early evidence of illness.
- Supply and administer drugs, treatments and appliances under patient group direction following discussion with senior colleagues.
- Prescribe drugs and medicines within an independent/supplementary role in accordance with legislation if qualified to do so.
- Offer a complementary service to that of other team members within the practice.
- Act as an advocate for the patient at all times
- Demonstrate a clear understanding of legal and ethical responsibilities, professional accountability and responsibilities of the role.
- Ensure equipment is maintained and monitored in accordance with manufacturers advice and local policies and guidelines.
Training and Education
- Demonstrate a positive commitment to the training and assessment of all learners within the clinical area, and facilitate learning opportunities for other health care professionals.
- Contribute to the adoption of best practice through evidence, clinical benchmarking and guidelines.
- Develop and maintain own clinical and professional expertise by participation in clinical supervision, personal and professional development planning and within scope of practice and NMC code of conduct.

- Provide supervision for students and health care assistants as appropriate.

- Maintain accurate and comprehensive documentation in accordance with NMC and local policy for standards of record keeping.

- Promote the development of clear systems of communication and referral both within the practice and with other health and social care providers as required.
- Maintain patient confidentiality as per GLMC, national, NMC and Caldicott guidelines.

- Effective utilisation of information technology and clinical systems following agreed training.
- Demonstrate professional skills in facilitating effective and clear communication in dealing with sensitive and volatile situations, i.e. safeguarding, vulnerable patients.
- Utilize a variety of systems to communicate clinical information in accordance with the data protection act; fax, email, telephone, letters.
- Utilize personal skills to facilitate effective and clear communication with patients to ensure collaborative healthcare and concordance with recommended advice, treatment pathways and therapeutic interventions.
Research and Audit
- Participate in clinical audit as required.
- Promote a research culture and actively encourage the utilization of evidence, research and audit to develop practice.
- Utilize evidence to provide effective and appropriate care to clients.
Managerial and Leadership
- Demonstrate commitment to multidisciplinary team working across the local health economy.
- Contribute ideas for service development to the management team.
- Contribute to the development, implementation and monitoring of policies, procedures and clinical guidelines as appropriate.

- Supervise health care assistants and phlebotomist within the practice.
- Promote and develop professional nursing practice in accordance with legislation and ongoing local and national requirements.

Qualifications
Essential
- Registered nurse
- Current NMC registration
- Diploma/degree or ability to demonstrate evidence of study at level 3
Desirable
- Awareness of current health care policies and agendas

- Current evidence based guidelines for primary care
- Ear care, phlebotomy, health promotion and family planning, cervical cytology skills desirable but training could be provided
- Good communication skills
- Experience in wound care, management of acute and chronic wounds
