Job description

The Provide Stroke Rehabilitation Service supports a well‑established Early Supported Discharge Service and 25 inpatient stroke beds. We work with acute stroke units, social care and the Stroke Association to deliver high‑quality, multi‑disciplinary stroke rehabilitation that encourages staff participation and promotes service and career development.

Responsibilities
  • Manage a complex caseload of stroke patients, undertaking assessment, goal setting and physiotherapy treatment.
  • Deliver client‑centred, goal‑oriented rehabilitation programmes using evidence‑based practice, standardised assessment and outcome measures.
  • Provide supervision, annual appraisal and support to junior staff, therapy assistants and students.
  • Participate in service development, audit and review of the stroke service, spearheading clinical projects and ESD coordination.
  • Act as the key worker for discharge planning and post‑discharge rehabilitation, collaborating with multi‑disciplinary and voluntary sector teams.
  • Comply with HCPC standards of proficiency and keep up‑to‑date with professional CPD.
  • Contribute to the collection and maintenance of service records on the IT database in accordance with local policies.
Qualifications
  • Professional UK clinical registration (Physiotherapy) and proof of HCPC registration.
  • A degree in Physiotherapy, with knowledge of NICE clinical guidelines for stroke care.
  • Sound understanding of neuro‑anatomy and clinical presentations of stroke and impact on mobility and function.
  • Experience in a stroke rehabilitation setting and proficiency in assessment techniques (e.g., ISM, 6‑MWT, TUG, NFGR).
  • Ability to lead multidisciplinary collaboration and service development.
  • Excellent communication, teamwork and advocacy skills.
