Job Description
Summary

The Home Health Physical Therapist Assistant (PTA) performs treatments, provides exercise instruction, and patient education aimed at improving and enhancing the patient's well-being, following the established plan of care.

Responsibilities
  • Under the supervision of a Physical Therapist, adhere to all state-specific laws governing physical therapy in home care, follow the treatment plan as defined by the Physical Therapist, and perform only those procedures for which qualified and trained. Make recommendations for changes to the plan of care to the supervising PT.
  • Instruct and assist patients in evidence-based treatments within the scope of practice for a physical therapy assistant, according to state laws.
  • Observe, document, and report to the supervising PT, nurse, or physician the patient's response to treatment and any changes in condition. Coordinate care with healthcare team members as appropriate.
  • Educate patients, families, caregivers, and other healthcare team members about therapy within the scope of the PTA.
  • Visit patients across the agency's service area and nearby LHC agencies as needed.
  • Complete progress notes for each visit and submit them according to LHC policy.
  • Participate in staff meetings and committees as requested.
  • Adhere to LHC Therapy Standards of Practice.
  • Participate in weekend rotations to provide services as needed.
  • Perform other duties as assigned.
Education and Experience

License Requirements:

  • Current license, registration, or certification to assist in physical therapy practice in Florida.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to a reliable vehicle.
